Washington County Healthy Communities Survey
Think for a moment about the kind of things in your community that make it easier for people to be healthy.
Through a grant from the Health Promotion Center of Glens Falls Hospital, the Healthy Communities Coalition of Washington County and the Adirondack Rural Health Network will be looking at some of the things that might make is easier for people to eat healthier and increase their physical activity.
Our first step is to have anyone who lives, works, plays and learns in Washington County to have the opportunity to participate in this short Healthy Communities Survey.
This survey is part of a planning process that may lead us towards local policy and environmental changes that make the healthy choice the easy choice.
By participating in the survey you can enter a drawing for one of six $100.000 gift cards to Dicks Sporting Goods. Your name will not be associated with your survey and we would only use your phone number to let you know if you have won.
