The Survey Data is comprised of the survey data collected by the Adirondack Rural Health Network (ARHN) in cooperation with County Health Departments in 2004 and 2007 (for two additional counties). The survey approach was a random telephone survey of approximately 300 households in each county.

The Community Health Assessment Data (CHA Data) is comprised of county-level health data provide by the NYS Department of Health and other agencies. The most recent county health data in database is for the year 2004.
